It is written in to the code to do this, unsure at this moment why it fails silently. Will do some digging.

Both of these should be fixed in the next version.
The double notification came when you "follow" a user, it sent a second notification which was superfluous.
I think I can mark this as resolved, but if you come across any more instances of this, please let me know and I'll reopen the bug,
